Candidate must have a current, active, and unrestricted licensure or certification in the candidate’s primary practice within the medical, health care, or rehabilitation field.

Candidate must have a minimum of two years of full-time paid employment within the primary practice of medical, health care, or rehabilitation field.

Completion of FIG’s 120-hour Life Care Planning Certification Class, which includes submission of a redacted/case study Life Care Plan to be peer-reviewed by the FIG Advisory Board with a passing score.
